Job Description- Role:  Care Coordinator

Location:  On-site – 6019 S 66th E. Ave. Suite 200 Tulsa, OK 74145

Relationship: This position reports to the Director of Operations

 

Our Care Coordinators help our community by matching our clients and caregivers together. They work with the local team to ensure our clients have the best matched care and act as a case manager for ongoing needs of the clients.

  • Builds relationships with clients and their families to match them with caregivers who can provide the needed services.
  • Build relationships with caregivers to match them with clients and assignments that fit their needs and improves retention. 
  • Complete monthly check-ins with clients, ensuring they are satisfied with the services we are providing. 
      • Check-ins will be a mixture of text, email, phone calls and in-person visits depending on the situation.

  • Create a positive, thorough, and welcoming onboarding experience for new clients. 
  • Keep scheduling system up to date and accurate with client schedules, contact information and preferences.

  • Participates in on-call duties on a rotating basis and as needed by the office.
  • Works closely with the Talent Specialist to create a successful, supportive and on-going experience to our clients and Caregivers.

 

Our Care Coordinators all have the below skills or qualities, ensuring success at Griswold:

  • Associate degree in human services or related field is preferred but not required.
  • Previous experience in the home care industry, human services, or a similar field required (or a strong desire to help others). 
  • Ability to pass all hiring criteria as defined by Griswold standards and/or state regulations, along with reliable transportation, valid driver’s license, and valid car insurance.
  • Must be able to operate standard business equipment including a laptop computer and a mobile phone as well as all the associated apps and software. 
  • Must be able to see and interpret information within our systems (like WellSky, FranConnect or Email) and be able to exchange information accurately and quickly with others, as well as record information in these same systems. 
  • Must be able to lift up to 40 pounds and alternate from stationary to traverse positions often and for extended periods of time.
  • Strong verbal and written communication skills 
  • High level of emotional intelligence and empathy, as well as, dedication to our core pillars 
  • Creative problem-solving skills 
  • Organized and detail oriented 
  • Flexible and able to multi-task 
  • Effective time management skills 
  • Proactive with strong initiative and follow through 
  • Capable of working independently when needed 
  • Weekend and/or evening hours may be required
