We are seeking a dependable and experienced Licensed Practical Nurse (LPN) to work alongside our Director of Client Services in supporting the ongoing assessment and care management needs of our clients.

This is an ideal opportunity for an LPN who enjoys working independently and is looking for a flexible, per diem schedule. The LPN will conduct in-home client reassessments, help ensure client records remain current and complete, and support the Director of Client Services with ongoing client documentation and compliance requirements.

The LPN will have significant flexibility in determining their schedule and will coordinate with the Director of Client Services to establish when client reassessments need to be completed.

 

Key Responsibilities

  • Conduct in-home client follow-up visits and reassessments as permitted within LPN scope of practice and Maryland RSA requirements, under the direction of the RN.
  • Coordinate with the Director of Client Services to identify clients due for reassessment.
  • Independently schedule and map out client visits based on reassessment needs and availability.
  • Assess changes in a client's condition, functional abilities, care needs, and overall situation.
  • Review existing care plans and identify when updates or modifications may be appropriate.
  • Document assessment findings accurately and promptly.
  • Ensure client files are properly updated following each assessment.
  • Review documentation for completeness and communicate any outstanding information or concerns to the Director of Client Services.
  • Identify changes in client needs that may require additional services, caregiver support, or follow-up.
  • Maintain appropriate communication with the Director of Client Services regarding client status and assessment findings.
  • Support the agency's efforts to maintain accurate, organized, and compliant client records.
  • Provide limited clinical documentation and quality-assurance support related to client assessments as mutually agreed upon.

 

Schedule & Flexibility

No set weekly schedule. Reassessments are assigned based on due dates, and the LPN coordinates directly with clients to arrange visits around their own availability.

The LPN will have the ability to:

  • Set their own assessment schedule in coordination with client availability.
  • Determine when and how to group client visits geographically.
  • Schedule visits based on the reassessment needs of the agency.
  • Work as many or as few assessment visits as mutually agreed upon.
  • Complete assessments during mutually convenient daytime hours.

This position may be particularly well suited for an LPN seeking supplemental income, a flexible work schedule, or an opportunity to remain engaged in clinical practice without a traditional full-time schedule.

 

Qualifications

  • Current and unrestricted Maryland LPN license.
  • Previous experience in home care, home health, geriatric care, case management, or a related setting preferred.
  • Experience conducting client assessments or reassessments preferred.
  • Strong clinical assessment and documentation skills.
  • Ability to work independently and manage a flexible schedule.
  • Strong organizational and time-management skills.
  • Professional communication and interpersonal skills.
  • Reliable transportation and ability to travel to client homes throughout assigned service areas.
  • Comfortable working with older adults and their families.
