What You’ll Do

The Integrated Behavioral Health Therapist (IBHT) works collaboratively with the Primary Care team to provide brief behavioral health support and short-term counseling within the primary care setting. This role helps patients of all ages address behavioral health concerns, strengthen coping skills, and improve self-management.

  • Provide brief, targeted behavioral health interventions and short-term counseling using evidence-based approaches.
  • Assess behavioral health needs, readiness for change, barriers to care, and treatment adherence while helping patients establish achievable goals.
  • Support patients and families experiencing concerns such as grief, caregiver stress, substance use, new medical diagnoses, and other behavioral health conditions.
  • Collaborate with providers, RN Case Managers, and the care team to coordinate treatment, identify patients needing additional services, and connect them with appropriate community resources.
  • Respond to crisis situations appropriately and maintain timely, accurate documentation in accordance with professional and regulatory standards.
  • Perform a variety of tasks and other duties as assigned.

 

Who We’re Looking For

We’re looking for a compassionate behavioral health professional who enjoys working as part of a collaborative Primary Care team and meeting patients where they are.

  • Master’s degree in a recognized mental health profession with a current Colorado professional license in good standing. LCSW or LPC preferred. Candidates actively working toward licensure must be able to obtain licensure within 2 years.
  • Current American Heart Association BLS certification within 3 months of initial employment and maintain current status throughout GRHD employment.
  • 2+ years of experience in an integrated care or clinical setting preferred, with experience providing brief behavioral health interventions, particularly for patients managing chronic conditions.
  • Experience working with diverse populations, including individuals from varied cultural, socioeconomic, and educational backgrounds. Experience working with immigrant and refugee populations preferred.
  • Bilingual Spanish/English skills preferred.

Compensation may vary outside this range depending on a number of factors, including a candidate’s qualifications, skills, competencies and experience.
