Admissions Specialist

Job Summary

Elite Home Care, Day Centers & Transportation is seeking a compassionate, organized, and detail-oriented Admissions Specialist to serve as the first point of contact for individuals and families exploring our Home Care, Adult Day Health, and Transportation services.

The Admissions Specialist is responsible for managing incoming inquiries, completing initial intake and qualification, identifying potential funding pathways, coordinating next steps, and maintaining consistent follow-up throughout the admissions process. This position plays an important role in ensuring every prospect receives timely, accurate, and compassionate guidance from their first interaction with Elite through enrollment or appropriate handoff.

The ideal candidate is a strong communicator who is comfortable managing a high volume of leads, understands healthcare funding and referral processes, and can confidently guide families who may be unfamiliar with available services or how to access them.

Key Responsibilities

  • Serve as the primary point of contact for incoming prospective client and family inquiries by phone, email, website, and referral sources.

  • Respond to new inquiries promptly and maintain consistent follow-up throughout the admissions process.

  • Complete intake and qualification, documenting service needs, location, referral source, funding source, and other relevant information.

  • Educate families on Elite's Home Care, Adult Day Health, and Transportation services and explain the appropriate next steps for enrollment.

  • Identify potential funding pathways, including Medicaid Waivers/CLTC, OIDD programs, VA benefits, Medicare-supported programs, private pay, long-term care insurance, and other available resources.

  • Coordinate appropriate next steps, including Day Center tours, Home Care assessments/sign-ups, and internal handoffs.

  • Maintain ownership of prospects through the admissions process and ensure no inquiry or follow-up is missed.

  • Maintain accurate, real-time documentation of leads, follow-up activities, status changes, referral sources, and outcomes within the CRM and other required systems.

  • Work closely with Regional Marketing Specialists and operational teams to ensure a smooth transition from referral to admission.

  • Communicate barriers to admission, trends, referral opportunities, and unresolved prospects to the Director of Sales & Marketing.

  • Participate in regular Admissions and Sales & Marketing meetings and provide updates on lead volume, conversion activity, and pipeline status.

  • Maintain a high level of professionalism, compassion, and confidentiality when communicating with families, referral partners, and internal teams.

Qualifications

  • Minimum of 2 years of experience in healthcare admissions, healthcare sales, customer service, case management, social services, or a related field preferred.

  • Knowledge of healthcare funding sources, including Medicaid Waivers, Community Long Term Care (CLTC), Medicare programs, OIDD services, VA benefits, and private-pay services preferred.

  • Strong phone and communication skills with the ability to explain complex services and funding processes in a clear and compassionate way.

  • Strong follow-up skills and the ability to confidently manage a high volume of prospects at different stages of the admissions process.

  • Excellent organization and attention to detail in a fast-paced environment.

  • Ability to collaborate across multiple departments while maintaining clear ownership of assigned responsibilities.

  • Experience using CRM systems and Google Workspace preferred.

  • Healthcare, case management, admissions, or social services experience is strongly preferred.

Schedule

Full-time, primarily Monday through Friday. Some flexibility may occasionally be required based on business and admissions needs.
