Qualified applicants who successfully complete our hiring process and meet all New York State and agency requirements will be considered for available client assignments as they become available based on client needs, scheduling, qualifications, and geographic location.

Responsibilities

• Provide hands-on personal care, including bathing, dressing, grooming, toileting, and mobility assistance while preserving each client's dignity and independence.

• Assist clients with activities of daily living, including meal preparation, feeding, light housekeeping, and medication reminders.

• Monitor clients for changes in condition and promptly report concerns to the supervising nurse or office staff.

• Provide companionship and emotional support while encouraging client engagement and independence.

• Maintain accurate documentation of care provided and client status.

• Follow each client's individualized Plan of Care and comply with all agency policies, infection control practices, and safety procedures.

Requirements

• Current New York State Home Health Aide (HHA) Certification.

• Must have worked for a licensed home care agency within the past two (2) years or otherwise meet New York State eligibility requirements.

• Compassionate, dependable, and committed to providing outstanding care.

• Strong communication skills and the ability to follow individualized care plans.

• Reliable transportation to travel throughout the Riverhead and Southold service areas.

• Ability to successfully complete all agency hiring requirements, including background screening, employment verification, and required health clearances.
