Ganado Nursing and Rehabilitation Center, part of Creative Solutions, is seeking a dedicated PRN Physical Therapist (PT) to join our in-house rehabilitation team in Ganado, Texas. Position Summary
As a PRN Physical Therapist, you will provide therapy services to residents to help them regain mobility, improve function, and achieve their highest level of independence. You’ll work closely with the interdisciplinary team to ensure optimal outcomes for each patient.

Key Responsibilities
• Evaluate residents to determine therapy needs and goals.
• Develop and execute individualized treatment plans.
• Deliver skilled therapy interventions to restore strength, balance, and mobility.
• Maintain accurate and timely clinical documentation.
• Communicate effectively with the care team, residents, and families.
• Adhere to all facility, state, and federal policies and guidelines.

Qualifications
• Active Texas Physical Therapist license
• In good standing with all licensing and regulatory agencies
• Experience in Skilled Nursing Facilities (SNF) or long-term care preferred
• Excellent communication and teamwork skills
• Familiarity with Net Health documentation system a plus
