Honey Grove Nursing Center is a 102-bed Medicare and Medicaid–certified skilled nursing facility located at 1303 East Main Street, Honey Grove, TX. The center provides individualized, person-centered rehabilitation and care, supporting both short-term recovery and long-term health needs.

Position Overview

We are seeking a licensed Physical Therapist to join our rehabilitation team on a per diem (PRN) basis. You will provide clinical services to residents in a supportive, in-house rehab setting.

Key Responsibilities

• Consult with residents to assess their physical condition and functional capacity

• Interpret evaluations and test results to inform treatment planning

• Develop and execute personalized rehabilitation programs using various techniques

• Deliver prescribed physical therapy treatments to relieve pain, restore function, and support healing

• Monitor patient response and modify treatment plans in collaboration with the interdisciplinary team (IDT)

• Provide education and clinical guidance to residents, families, and caregivers

• Maintain timely and accurate documentation in compliance with company and state/federal regulations

• Participate in team meetings as directed by the Director of Rehab

Qualifications & Skills

• Active and valid Physical Therapy license in Texas

• Skilled Nursing Facility (SNF) or long-term care experience strongly preferred

• Familiarity with treatment best practices and safety protocols

• Experience with Net Health electronic documentation system is a plus

• Strong communication and interpersonal skills, and ability to work effectively with diverse patients and colleagues
