On-Call: One weekday per week + One weekend per month

 About the Role

Continuum Home Health Care is seeking a compassionate, detail-oriented Intake Coordinator to join our growing team. As the first point of contact for our clients and referral sources, you’ll play a vital role in ensuring that each new client receives the high-quality, person-centered care that sets Continuum apart.

You will work closely with our care team to verify eligibility, coordinate services, and make sure every client’s journey into care starts smoothly. This position is ideal for a team player who thrives in a fast-paced, collaborative, mission-driven environment.


 Key Responsibilities

  • Manage incoming referrals and complete initial intake assessments over the phone

  • Verify insurance and payer eligibility and secure authorizations when required

  • Enter accurate client data into our EMR system (AxisCare)

  • Coordinate and schedule start-of-care visits with the clinical team

  • Communicate clearly with families, caregivers, case managers, and providers

  • Track and follow up on pending referrals to ensure timely care delivery

  • Participate in weekly scheduling meetings and quality checks

  • Handle one weekday of on-call support per week and one weekend per month

  • Support the team with additional administrative duties as needed


 What We’re Looking For

  • Experience: Minimum 2 years in a home care, healthcare office, or intake/scheduling role

  • Technology: Proficient in EMR systems (preferably AxisCare), Microsoft Office, Google Docs, and Monday.com

  • Personality: Friendly, professional, dependable, and able to work under pressure

  • Team Mindset: We value collaboration. We’re looking for someone who can jump in, problem-solve, and support the team with a positive attitude.

  • Detail-Oriented: You’re known for triple-checking your work and staying organized

  • Communicator: Clear and compassionate communication is a must—with families, staff, and referral sources
