About Open Arms Solutions

Open Arms Solutions is a premier private-duty home care provider dedicated to helping seniors live safely, independently, and with dignity in the comfort of their own homes.

We are looking for a compassionate, proactive, and highly organized Community Care Coordinator to support our clients, families, and caregivers throughout the western suburbs, including Oak Brook, Naperville, and surrounding communities.

About the Role

The Community Care Coordinator is a key member of our operations team and reports directly to the Executive Director. This position owns the client experience from initial assessment through ongoing care coordination while also supporting caregiver performance, communication, and retention.

The ideal candidate combines strong relationship skills with the ability to solve problems, manage multiple priorities, and respond quickly when client or caregiver needs change.

Key Responsibilities

  • Conduct client consultations and assessments and develop individualized care plans.
  • Guide clients and families through intake, care planning, service options, and budgeting.
  • Manage a designated client caseload and serve as the primary point of contact for clients and families.
  • Monitor client satisfaction and proactively identify changing care needs.
  • Conduct regular quality assurance visits and client/family check-ins.
  • Partner with scheduling to ensure appropriate caregiver coverage and continuity of care.
  • Match caregivers with clients based on skills, experience, personality, and client preferences.
  • Support caregiver hiring, onboarding, training, coaching, and performance.
  • Help resolve client, family, caregiver, and scheduling concerns quickly and professionally.
  • Provide emergency client-shift coverage when necessary.
  • Maintain accurate and confidential client and caregiver documentation.
  • Participate in process improvement and technology initiatives, serving as a super-user of key business systems.

Qualifications

  • 3–5 years of experience in senior care, private-duty home care, care management, hospitality, or another high-touch client service environment.
  • Private-duty/home care experience strongly preferred.
  • LPN certification strongly preferred.
  • Experience managing client relationships and supporting professional caregivers.
  • Knowledge of senior-care ADLs and IADLs.
  • Excellent communication, organization, and problem-solving skills.
  • Strong judgment, professionalism, empathy, and attention to detail.
  • Experience with WellSky or similar care-management software and Microsoft 365 preferred.
  • Valid driver's license and reliable, professional transportation.
  • Ability to travel throughout the assigned territory, up to approximately 50%.
  • Ability to lift up to 50 pounds.

What We're Looking For

We want someone who takes ownership.

Someone who builds trusted relationships with clients and families, supports caregivers, anticipates problems before they become emergencies, and understands that exceptional home care depends on exceptional communication and follow-through.

Why Join Open Arms Solutions?

At Open Arms Solutions, our team is just as important as the clients and families we serve. You'll join a collaborative organization where your ideas are valued, your contributions are recognized, and your work directly impacts the lives of seniors and their families.

If you're an experienced care professional who wants to combine client relationships, care coordination, caregiver leadership, and hands-on problem solving, we'd love to hear from you.
