JOB SUMMARY: The On-Call Licensed Practical is responsible for providing selected nursing care under the direction and general supervision of the On-call RN and/or the DON. Ensuring all services needed are identified, outline in the Plan of Care and delivered to hospice and home health patients and families after regular work hours, on weekends and holidays. May be assigned visits, admissions or LPN triage duties.

JOB-SPECIFIC DUTIES AND RESPONSIBILITIES:

  1. Performs all on-call visits listed on weekend schedule report as directed by DON.
  2. Contacts DON for clarification if needed on scheduled on-call visits.
  3. Also receives communication from on-call service and responds quickly.
  4. Follows up to assess the need for on-call visit by contacting patient, family, or facility for additional information.
  5. Reviews and follows Plan of Care for each assigned patient in conjunction with the Hospice or Home Health team, patient, family and attending physician.
  6. Assures appropriate on-call services are performed and notifies necessary family, staff, doctors, DME, pharmacy, or facility.
  7. Reports to the attending physician any significant changes in the patient's condition and requests change of orders and updates the EMR.
  8. Ensures coordination of care across all settings.
  9. Responsible for pain and symptom control.
  10. Instructs patient and family on techniques of patient care.
  11. Participates in admission procedures as needed.

12. Reviews with patient and family the Home Health and Hospice benefit and all services that are available.

13. Completes all required documentation in a timely manner

14. Participates in weekly Monday morning report meetings to update clinical staff.

  1. Participates in monthly in-service training or education programs for staff.
  2. Complies with the state and federal regulatory requirements.
