The Resident Care Coordinator, LPN (RCC) ensures residents receive top-tier care and support tailored to their unique needs with compassion, consistency, and efficiency. By adhering to nursing best practices and regulatory standards, they deliver nursing services in alignment with the Nurse Practice Act, Federal, and State regulations, under the guidance of the Resident Care Director. The Resident Care Associate is a Culture Ambassador who will model the core values of the company, leading by example, and ensure all team members exemplify the core values of the company. Position: Full- Time, 2nd Shift 3pm -11pm , Monday- Friday
Qualifications
• Graduate from a state-approved school of practical nursing or registered nursing, an LPN license is required.
• Minimum one year of nursing experience, ideally including medication administration and expertise in geriatric, psychiatric, or rehabilitation nursing.
• CPR and First Aid certification, or willingness to obtain according to company policy.
• Upholds resident confidentiality.
• Flexible availability, including weekends, to ensure resident care and services.
• Willingness to acquire state-specific Individual Service Plan certification as necessary to meet regulatory requirements.
• Demonstrates sincere compassion and genuine concern for the well-being of seniors and their families.
• Proficient in using basic office equipment, including personal computers, to maintain high job performance.
• Aligns with Commonwealth Senior Living's mission of improving the lives of seniors, their families, and each other.
• Holds an acceptable driving record.
Areas of Primary Responsibility
• Provide direct nursing care and clinical oversight to nursing assistants working with residents.
• Foster continuity of care and effective communication by promptly reporting care provided and changes in residents' conditions to the Resident Care Director.
• Administer and meticulously document medication and treatments as per physician orders, ensuring accurate record-keeping of all resident care.
• Assist residents with lifts, moves, and transport, employing proper body mechanics or lifting devices for accident prevention.
• Responsively address inquiries from residents, visitors, and staff within established time frames and policy guidelines.
• Performs care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents.
• Document daily care activities in accordance with company policy and regulatory standards.
• Demonstrate familiarity with residents' individualized care plans and provide support, accordingly, contributing observations and insights to the care planning process.
• Communicate effectively and tactfully with residents, visitors, families, peers, and supervisors, cultivating a homelike environment.
• Uphold residents' rights, promptly reporting incidents or evidence of abuse or rights violations to the appropriate authorities.
• Perform driving duties as required (valid driver's license mandatory).
• Execute all job responsibilities in accordance with prescribed safety and infection control protocols, including thorough handwashing, proper use of disposable gloves, and appropriate disposal of soiled materials.
• Conduct various tasks assigned by the charge nurse, such as vital sign checks, weighing residents, applying creams/ointments, and specimen collection.
• Implement safety and infection control procedures, including proper hand hygiene and care techniques to prevent skin damage.
• Maintain a safe working environment through accident prevention and adherence to safety protocols.
• Attend all required training sessions, in-service programs, and staff meetings.
• Maintain regular communication with the Resident Care Director.
• Perform additional duties as assigned.

Physical/Sensory Requirements
The Resident Care Coordinator must be capable of lifting, stooping, carrying, turning, and assisting high-risk residents, as well as pushing medication carts. They must also be willing and able to work a flexible schedule. Additionally, they should be capable of continuous sitting, standing, bending, and movement throughout work hours, and able to lift and/or carry 50 to 75 pounds.
