Salary: $41,000 - $43,000 (A potential retention bonus may be offered in addition to the base salary.)

What You’ll Do:

  • Develop and maintain positive relationships with other Target Case Management Agencies and DMH staff.
  • Coordinate and/or participate in all scheduled conference calls. Update individual support plans (ISPs) and associated documentation. Initiate conference calls between other members in the state when a person is transferring between CCSOMO counties.
  • Assist with ensuring developmental disabilities consumers have services authorized, including meeting with consumers.
  • Prepare files and ensure all federal and state-mandated documentation is complete and all policies/procedures are followed.
  • Should there be an issue or potential reportable issue, notify the involved supervisor and DD Director for immediate action.
  • Attend or schedule inter-disciplinary team meetings to discuss client cases in relation to eligibility, support plans, progress, transfer progress, and possible changes in the service program.
  • Meet with program staff for complex residential moves. In addition, include staff in statewide phone calls as necessary.
  • Visit service agencies, attend meetings, and confer with other agency staff and state-designated staff to become informed concerning community resources in other parts of the state for persons with developmental disabilities.
  • Assist with training Support Coordinators about the transition process and requirements.
  • Performs other related duties as assigned.

Minimum Qualifications

Required:

  • Bachelor’s degree in social work (BSW) or equivalent human services degree; OR Registered Nurse (licensed)
  • This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.

Preferred:

  • Experience working with individuals with developmental disabilities as a case manager.
  • Knowledge of social services.
  • Equivalent combinations of relevant education and experience may be substituted as appropriate.
