Position Summary:

CareSense Home Care is seeking a compassionate and skilled Registered Nurse (RN) to provide in-home skilled nursing. This is a part-time field-based role ideal for a nurse looking for flexibility, autonomy, and rewarding patient care.

Essential Responsibilities:

• Perform initial and ongoing nursing assessments in participants’ homes.
• Develop and implement care plans in accordance with physician orders and participant goals.
• Administer skilled nursing care such as wound care, medication administration, and catheter care.
• Monitor patient health status and report significant changes to the physician and care team.
• Educate participants and caregivers on condition management and treatment compliance.
• Complete timely, accurate documentation per Electronic Visit Verification (EVV) standards in the EMR system (e.g., HHAeXchange).
• Communicate and coordinate with case managers, home health aides, and interdisciplinary team members.
• Participate in case reviews, care conferences, and required trainings as assigned.

Qualifications:

• Graduate of an accredited nursing program (ADN or BSN) (required)
• Current, unencumbered Pennsylvania Registered Nurse (RN) license (required)
• Current CPR certification (required)
• At least one (1) year of experience in home health, geriatrics, or chronic care (preferred)
• Reliable transportation, valid driver's license, and current auto insurance (required)
• Experience with Medicaid waiver programs and HHAeXchange is a plus (training provided)
