Social Services Director

Springs Village Care Center is seeking a dedicated and compassionate Social Services Director to lead our social services team. The ideal candidate will be responsible for overseeing the planning, coordination, and implementation of social services programs that enhance the well-being of our residents. This role requires strong leadership skills, excellent communication, and a commitment to providing high-quality care in a supportive environment.

Responsibilities

• Develop, implement, and monitor social services programs to meet the needs of residents.
• Lead and supervise the social services staff, providing guidance, training, and support.
• Collaborate with interdisciplinary teams to coordinate resident care plans and ensure compliance with regulatory requirements.
• Conduct assessments and develop discharge planning strategies to facilitate residents' transitions.
• Maintain accurate and timely documentation of social services activities and case records.
• Serve as a liaison between residents, families, healthcare providers, and community resources.
• Stay current on social services trends, regulations, and best practices.

Requirements

• Bachelor’s degree in Social Work, Psychology, Sociology, or related field (Master’s preferred).
• License or certification as required by the state of Colorado.
• Minimum of 3 years’ experience in a social services leadership role, preferably in a healthcare or long-term care setting.
• Strong leadership, interpersonal, and communication skills.
• Ability to work effectively with diverse populations and multidisciplinary teams.
• Knowledge of relevant laws, regulations, and ethical standards.
