Triage Nurse (RN)

Hospice Triage Nurse RN

Salary: $80-89k/yr BOE

Schedule: Monday- Thursday | 5 PM- 8 AM

Location: On-Site (Blue Ash Office)

Requirements of the Triage Nurse: 

• Licensed Registered Nurse in the state of practice 

• Hospice/ Crisis Care experience is REQUIRED 

• BLS Certified

• Willing to train in Cincinnati and Dayton

• Excellence in communication and interpersonal skills to build rapport with patients and families.

• Effective oral communication skills to interview patients and explain next steps as well as to pass information on to other members of the care team

• Familiarity with medical records, word processing, and email software programs

About the Triage Nurse role:

• Answer triage calls during assigned shifts while maintaining a calm, reassuring presence during emotionally intense situations.

• Assessing patients, symptoms, pain levels, and other concerns over the phone to determine the urgency of care and the need for in-person visits.

• Prioritize and respond to urgent calls promptly.

• Guiding on managing hospice-related symptoms like pain, nausea, and difficulty breathing, recommending appropriate medication adjustments or interventions.

• Assists with workflow and scheduling of visits

• Collaborating with the interdisciplinary team, including physicians, case managers, and other hospice staff, to ensure seamless care coordination and follow-up.

• Educating patients, families, and caregivers on the dying process, end-of-life care, and symptom management techniques.

• Accurately documenting all patient interactions, interventions, and follow-up actions in the electronic medical record.

• Update patient health information systems with notes

• Use clinical judgment to evaluate symptom severity and urgency, and triage patient and caregiver calls to determine the appropriate level of care.

• Provide emotional support and education to families and caregivers over the phone.

• Assist caregivers with crisis management and problem-solving.

• Accurately document all interactions, assessments, interventions, and follow-ups.

• Ensure compliance with state, federal, and hospice agency regulations and protocols.

• Any other tasks assigned.
