RN Case Manager – Houston
 
Private Duty In-Home Care: Cambridge Caregivers is a family-owned and operated provider of non-medical caregiver services. We are known for personalized attention and our great staff. Our caregivers are W-2 employees who have completed extensive training developed by our Director of Nursing. We have 300+ Caregivers, CNA’s, LVN’s, RN’s and EMT’s on staff in Dallas, Ft. Worth and Houston.

Registered Nurse Responsibilities: 

  • Perform comprehensive patient assessments and document findings 

  • Monitor and record vital signs (blood pressure, temperature, heart rate, etc.) 

  • Administer medications as ordered 

  • Provide wound care, including dressing changes and monitoring healing 

  • Educate patients and families on care plans, medications, and home care strategies 

  • Respond to patient concerns with professionalism and empathy 

  • Collaborate with Director of Nursing regarding changes in patient condition 

  • Perform ongoing patient data collection and maintain accurate documentation 

  • Develop, update, and follow individualized care plans 

  • Ensure all orders are followed, documented, and communicated appropriately 

  • Assist with emergency response measures when necessary 

  • Maintain a clean and safe environment 

  • Build trust and rapport with patients and families 

  • Deliver exceptional patient experience through responsiveness and care 

  • Serve as a resource and mentor to patient care staff 

  • Pursue ongoing professional development 

  • Support and uphold the mission, values, and philosophy of Cambridge Caregivers 

  • Adhere to all company policies, procedures, and standards of care 

 

Registered Nurse Requirements: 

  • Current RN license in the state of Texas and/or compact license 

  • Current CPR Certification (American Heart Association) 

  • Basic Life Support (BLS) Certification 

  • Reliable transportation 

  • Ability to work independently and follow clinical direction 

  • Compassionate, professional, and patient focused demeanor
