Home Health RN for Skilled Visits to service San Antonio and surrounding areas.

Full-Time $90k to $95k.

PRN $45.00 per visit.

RETENTION 10K

$400 ON CALL

A tablet will be provided for all Full-Time RNs's.

Travel Time and Mileage Paid for PRN

What You'll Do:

Conduct comprehensive assessments of patients' medical conditions and needs.Develop and implement individualized care plans in collaboration with the team.Administer medications, treatments and procedures as prescribed by physicians.Monitor patients' responses to treatments and make necessary adjustments to care plans.Maintain accurate and timely documentation of all patient interactions, treatments, and progress.Communicate effectively with patients, families, and other healthcare providers to ensure coordinated and optimal patient care.

Qualifications:

Current and valid Registered Nurse (RN) license in the state of Texas

CPR & BLS certification.Strong clinical skills and knowledge of home health care practices.Ability to work independently and manage time efficiently.
