Physical Therapist PT Responsibilities:

• Able to evaluate the patient's condition (muscle strength, gait, range of motion, and pain) developing an individualized plan tailored to based on the physician order.

• Work cooperatively with patient's family to develop a safe home environment appropriate for the patient well-being (especially as it relate to falls, interventions, assistive devices, or just instructing patient's family/caregiver on use of devices)  

• Report patient's response to treatment or changes in conditions to the Director of Nursing, patient's physician, and patient family   

• Initiate PT program, also instructing patient's family and caregiver in specific phases of patient treatment which may assist with patient's progress enhancing patient's strength, range of motion, flexibility, and minimizing discomfort and pain. 

• Prepare clinical notes and progress summaries, as well as participate in discharge planning(submitting a copy to the director of nursing) 

Physical Therapist PT Requirements:

• Current unrestricted PT license in state for which care is being provided

• Certified CPR and negative TB skin test or chest x-ray

• Minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility, preferred.

• Licensed driver with valid automobile insurance

• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull(varied assistance is required depending on the mobility of the client) 

• Must be able to lift up to 50 pounds
