Case Details!

• 85 yr old male with dementia, and needs COMPLETE assistance
• Start at 3pm and end at 8pm (5 hours)
• Mon-Fri
• Needing help w/ALL ADLS, uses a wheelchair. He is on a dysphagia diet, needs thickened liquids. Can become agitated at times. Needs to be redirected and reoriented.

Your (CHHA) Daily Duties!

• Companionship & Conversation
• Personal Care (bathing, dressing, grooming, etc.)
• Ambulation & Transfers
• Meal Prep / Med Reminders
• Walk, Stand, Lift & Carry up to 45lbs. for extended periods of time
• Assisting client with active and passive mobility work defined by the plan of care, and documenting observed changes to the Director of Nursing
