The RN Client Care Coordinator is responsible for supporting the clients and field staff to ensure that the plan of care is being followed, providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes. Responsibilities
- Supporting the clients and field staff and coordinating clinical care
- Assist with Client Assessments and Care Plan development
- Ensure that plan of care and treatment is being followed, using sound judgment and critical thinking
- Provide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process
- Prepare, administer, and appropriately document actions taken specifically to the commonly prescribed oral, topical, subcutaneous, and intramuscular medications. Observe, report, and record adverse reactions to medications and/or treatment to the registered nurse
- Provide direct client care as needed
- Perform all skills, treatments, and procedures to the scope of practice as defined in the Nurse Practice Act in the state of clinical practice and adheres to the policies, procedures, and protocols of the agency
- Treat clients, family, and staff with courtesy and respect
- Assists with the intake of new clients and inform potential clients of our services and reasons to utilize BrightStar Care of Honolulu over other agencies
- Assists in the development of organization goals and agency performance improvement activities
- Administers agency policies and procedures
- Assists the Director of Nursing with assessing the competency of staff to promote quality, continuity, and safety of a client’s care
- Supervises educates and coaches other field personnel
- Conducts staff and patient training
- Share on-call and after-hours RN coverage
- Provide information to be used for statistics, reports, and records for purposes of evaluation and reporting of organization activities
- Assures proper maintenance of clinical records in compliance with local, state, and federal laws
- Assures compliance with all local, state, and federal laws
Requirements
- Graduated from an accredited school of nursing
- Unencumbered license as a Registered Nurse in the state of Hawaii
- Certified CPR and demonstrate strong clinical skills
- Flexibility and good time management skills A MUST
- Excellent organization, planning, and project management skills.
- Excellent customer service skills and experience
- Strong problem-solving abilities, puzzle-solving mindset
- Caring, empathetic, and tactful, able to connect with others
- Excellent verbal and written communication skills
- Unflappable - able to perform under pressure and time constraints
- Valid driver’s license; valid state-required proof of auto insurance.
- Negative TB test or chest x-ray
- Adhere to HIPAA and maintain client confidentiality
