Per Diem/ Part-Time PT needed $70 per visit shifts and $100 per assessments Monday- Friday 8am-5pm. We are looking for a Home Health Physical Therapist on a per diem/ part-time basis for our Maplewood territory. Come join BrightStar Care, a client-centric team that holds value in quality care and serving with passion. The Physical Therapist must be able to evaluate the patient’s condition and develop an individualized plan tailored to each client based on the physician’s orders. Help clients enhance strength, range of motion, flexibility, and minimize pain or discomfort. Work as much or as little as you want, with our highly flexible schedule.


  • 1. Utilizes problem solving, decision making methodology in evaluating, planning, communicating, directing and delivering patient care.


    A. Performs evaluations and discharges, determines appropriateness of treatment, clarifies orders as needed, and establishes PT plan of care.

    B. Provides direct PT services following standard procedures and safety precautions for the application and use of treatment modalities

    C. Provides training and education to patient, family and staff.

    D. Maintains required documentation

    E. Provides direction and supervision to Physical Therapist Assistant

    F. Performs screenings and pursues physician orders when appropriate

    G. Provides reports and recommendations for patient care conferences, discharge meetings,


    H. Obtains necessary authorization for PT services

    2. Participates in the development, revision, and implementation of programming for the physical therapy services.

    3. Contributes to the job performance evaluation process for PTAs

    4. Maintains an effective working relationship with fellow employees.

    5. Performs duties in a safe manner and observes approved safety procedures at all times.

    6. Participates in educational programs in the interest of improving skills as required, and for

    professional growth and development.

    7. Follows State and Federal regulations as they pertain to the practice of Physical Therapy.

    8. May perform reasonable similar or related duties as assigned or instructed.

    9. Willing to travel up to 30 miles for client/patient visits.

    10. Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.

    11. Must be fully vaccinated or be willing to complete full vaccination by date of hire and proof of vaccination will be required or comply to weekly COVID testing.


  • Education: Graduate of Physical Therapy Program, minimum of Bachelor’s degree
  • Experience: A minimum of one year experience as a physical therapist in a home health setting is preferred.
  • Special Knowledge, Skills and Abilities:
  • Licensed in the state of MN
  • Ability to work effectively with clients

  • Demonstrate knowledge of current insurance guidelines

  • Recommend proficiency with the use of PDFs, Word, Outlook, Excel programs.
