Part-Time Licensed Practical Nurse Hours Available Mondays 1p-7p, Thursdays and Sundays 7a-1p. Additional Per Diem Shifts available. 

Elderly Male, Progressive Dementia, Severe Fall Risk. Uses Wheelchair. Needs a nurse that makes a great first impression. Memory Care Experience a Must. Located in Downtown Chicago with Parking.

Licensed Practical Nurse (LPN) Job Duties:

• Perform care as defined by client’s individual Plan of Care and within scope of state’s nurse practice act.
• Monitor patient’s condition and notify appropriate personnel of any changes.
• Provide those services requiring substantial and specialized nursing skill.
• Assures proper maintenance of clinical records in compliance with local, state and federal laws.
• Counsels the client and family in meeting nursing and related needs.

Position Requirements 

Nurse Qualifications:

• Current and unrestricted LPN license in state for which you are applying. (Illinois)
• Home Health Experience Preferred
• Memory Care Experience Required.
• 1 year experience.
• Proof of eligibility to work in the US.
• Clean background and criminal record.
