With a total team approach, we combine dedicated administration with an experienced professional staff to offer a variety of services and programs for our residents, ranging for short term rehabilitation to long term and dementia focused care. We are looking for compassionate, hardworking people to join our team of dedicated caregivers.

Our Rehabilitation Unit Nurse is responsible for providing comprehensive care for our residents looking to rehab to home and/or rehab to our long term care units.  They working closely with our in house Therapy Team and our Skilled Services team to ensure resident goals are met or exceeded.  Currently this position has multiple available shifts including Day Shift 6:45am-7p and 6:45p-7a

Responsibilities

  • Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of residents
  • Assess residents’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review
  • Administer medication as prescribed by the resident’s physician
  • Provide effective communication to residents, their family members, team members, and other health care professionals

Qualifications

  • Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing
  • Current state license as an RN
  • Proof of current CPR certification 
  • Compassionate, friendly, team orientated demeanor 
  • Excellent communication skills
