A Patient Care Coordinator acts as a liaison between patients & healthcare providers while managing communication, coordination and scheduling across healthcare teams; ensuring patients receive organized, high‑quality care.

The Job:
 
  • Serve as the point of contact for all interactions with clinical team members, families, patients, and administrative associates in relation to care coordination.
  • Coordinate care schedules and referrals.
  • Provide an outstanding admissions experience for our patients.
  • Conduct follow-up calls and remind patients of upcoming appointments. 
  • Maintain accurate patient records and visit documentation in EMR system (Kinnser, HCHB). 
  • Update electronic medical records after visits, tests, and treatments. 
  • Maintain a tracking system to ensure that all visit frequencies are input for proper scheduling.
  • Work with leadership to ensure that all clinicians meet productivity.
  • Maintain clinicians' availability list.
  • Assist with insurance pre-certifications and ensure services are billed correctly.
  • Maintain positive working relationships with internal and external customers, while exhibiting the core values of Transition Home Healthcare.

Experience Required:

  • 1 year of experience with home health patient care coordination.
  • 1 year of experience with the following EMR systems: Kinnser, HCHB.
