Position Summary:
The Travel Home Health Therapist is responsible for evaluating and treating patients in the home setting in accordance with physician orders and individualized treatment plans. Therapists will be flexible in traveling throughout designated Houston service areas to support patient care needs and therapy coverage.
Responsibilities:
•Provide therapy evaluations and treatment services in patients’ homes
• Develop and implement individualized treatment plans
• Complete timely and accurate clinical documentation
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Educate patients and caregivers regarding treatment goals and home exercise programs
• Monitor patient progress and adjust treatment plans as appropriate
• Maintain compliance with company policies, Medicare guidelines, and healthcare regulations
• Travel to assigned patient locations throughout the Greater Houston & surrounding areas
Qualifications:
Current Texas license in applicable discipline (PT, PTA, OT, COTA, or SLP
• Home health experience preferred, but not required
• Strong clinical, communication, and organizational skills
• Reliable transportation and valid driver’s license
• Ability to work independently and adapt to varying patient needs
• Willingness to travel throughout assigned Houston territories
• Compassionate, dependable, and patient-focused mindset
