Job Description

We are seeking a compassionate Physical Therapist to join a long-term care/skilled nursing facility (LTC/SNF) in Lebanon, NH on a 13-week travel assignment. The Physical Therapist will provide rehabilitative services to residents, focusing on mobility, strength, and independence in a supportive care environment. The ideal candidate has prior LTC/SNF experience and is dedicated to delivering patient-centered care to geriatric populations.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations for residents in a skilled nursing/long-term care setting.
• Develop and implement individualized treatment plans tailored to residents’ needs.
• Provide therapeutic interventions including gait training, strengthening, mobility, and balance exercises.
• Educate residents, families, and caregivers on therapy goals, safe mobility strategies, and home exercise programs.
• Document evaluations, therapy sessions, and progress notes accurately in compliance with facility and regulatory standards.
• Collaborate with occupational therapy, speech therapy, nursing, and interdisciplinary teams to ensure coordinated care.
• Promote independence, mobility, and overall wellness through evidence-based PT practices.

Qualifications

• Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program.
• Current and valid PT license in New Hampshire (or compact eligibility).
• Minimum 1 year of recent PT experience; LTC/SNF experience preferred.
• Current certification in Basic Life Support (BLS).
• Strong clinical reasoning, documentation, and communication skills.
• Compassionate approach with experience treating geriatric and long-term care populations.
• COVID-19 vaccine and recent tuberculosis test.
