Physical Therapist – LTC/SNF – Travel Position

Pay: $2,435/week

Job Description

We are seeking a compassionate Physical Therapist to join a long-term care/skilled nursing facility (LTC/SNF) in Cowens, WV on a 13-week travel assignment. The Physical Therapist will help residents improve mobility, restore function, and enhance quality of life in a supportive long-term care environment. The ideal candidate has prior LTC/SNF experience and a strong commitment to patient-centered care.

Key Responsibilities

• Perform comprehensive physical therapy evaluations for residents in a skilled nursing/long-term care setting.
• Develop and implement individualized treatment plans tailored to residents’ needs.
• Provide therapeutic interventions such as gait training, strengthening, mobility, and balance exercises.
• Educate residents, families, and caregivers on safe mobility practices and therapy goals.
• Document therapy sessions, progress, and care plans accurately and in compliance with facility and regulatory standards.
• Collaborate with occupational therapy, speech therapy, nursing, and interdisciplinary teams to ensure coordinated care.
• Support independence, mobility, and overall wellness through evidence-based PT practices.

Qualifications

• Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program.
• Current and valid PT license in West Virginia (or compact eligibility).
• Minimum 1 year of recent PT experience; LTC/SNF experience preferred.
• Current certification in Basic Life Support (BLS).
• Strong clinical reasoning, communication, and documentation skills.
• Compassionate approach with experience treating geriatric and long-term care populations.
• COVID-19 vaccine and recent tuberculosis test.
