Physical Therapist – LTC/SNF – Travel Position

Job Description

We are seeking a compassionate Physical Therapist to join a long-term care/skilled nursing facility (LTC/SNF) in Scottsdale, AZ on a 13-week travel assignment. The Physical Therapist will help residents restore mobility, improve strength, and enhance overall quality of life within a supportive care environment. The ideal candidate has prior LTC/SNF experience and is committed to providing patient-centered rehabilitative care.

Key Responsibilities

• Perform comprehensive physical therapy evaluations for residents in a long-term care/skilled nursing setting.
• Develop and implement individualized treatment plans tailored to residents’ needs.
• Provide therapeutic interventions such as gait training, strengthening, mobility exercises, and balance training.
• Educate residents, families, and caregivers on safe mobility strategies and exercise programs.
• Document evaluations, treatment sessions, and progress notes accurately and in compliance with facility and regulatory standards.
• Collaborate with occupational therapy, speech therapy, nursing, and interdisciplinary teams to ensure coordinated care.
• Promote independence, mobility, and overall well-being through evidence-based PT practices.

Qualifications

• Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program.
• Current and valid PT license in Arizona (or compact eligibility).
• Minimum 1 year of recent PT experience; LTC/SNF experience preferred.
• Current certification in Basic Life Support (BLS).
• Strong clinical reasoning, communication, and documentation skills.
• Compassionate approach with experience treating geriatric and long-term care populations.
• COVID-19 vaccine and recent tuberculosis test.
