Physical Therapist – Long-Term Care/SNF - Travel Position

Job Description

We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Nags Head, NC, on a travel contract. The Physical Therapist will evaluate residents, develop personalized treatment plans, and provide therapy services to improve strength, mobility, balance, and independence. The ideal candidate is patient-focused, adaptable, and experienced in geriatric rehabilitation.

Key Responsibilities

• Conduct physical therapy evaluations to assess residents’ functional abilities, mobility, and therapy needs.
• Design and implement individualized treatment plans in collaboration with physicians and interdisciplinary staff.
• Deliver therapy interventions including strengthening, gait training, balance exercises, and functional mobility activities.
• Educate residents, caregivers, and families on therapy goals, safe mobility practices, and use of assistive devices.
• Accurately document evaluations, therapy sessions, and progress in compliance with facility and regulatory standards.
• Collaborate with occupational therapy, speech therapy, and nursing staff to ensure coordinated care.
• Support residents in regaining independence and enhancing quality of life through evidence-based therapy practices.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in North Carolina (or compact license).
• Minimum of 1 year of recent PT experience, preferably in LTC/SNF settings.
• Current certification in Basic Life Support (BLS).
• COVID-19 vaccine and recent tuberculosis test.
• Strong clinical reasoning, documentation, and communication skills.
• Compassionate, resident-centered approach with geriatric rehabilitation experience preferred.
