Job Description
We are seeking a dedicated Physical Therapist to join a skilled nursing and long-term care facility in Santa Fe, NM, on a travel contract. The Physical Therapist will evaluate residents, create individualized care plans, and deliver therapy to improve mobility, strength, balance, and overall quality of life. The ideal candidate is compassionate, adaptable, and experienced in geriatric rehabilitation.
Key Responsibilities
- Perform initial evaluations to determine residents’ physical function, mobility, and therapy needs.
- Develop and implement customized treatment plans in collaboration with physicians and interdisciplinary staff.
- Deliver therapeutic interventions including gait training, strengthening, balance activities, and functional mobility.
- Educate residents, families, and caregivers on therapy goals, safety practices, and assistive device usage.
- Accurately document evaluations, progress notes, and therapy sessions in compliance with facility and regulatory standards.
- Work collaboratively with occupational therapy, speech therapy, and nursing staff to ensure holistic, patient-centered care.
- Support residents in achieving independence and improved quality of life through evidence-based therapy practices.
Qualifications
- Doctorate or Master’s degree in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in New Mexico (or compact license).
- Minimum of 1 year of recent PT experience, preferably in LTC/SNF settings.
- Current certification in Basic Life Support (BLS).
- COVID-19 vaccine and recent tuberculosis test.
- Strong communication, assessment, and documentation skills.
- Compassionate, resident-centered approach with geriatric rehabilitation experience preferred.
