Physical Therapist LTC/SNF – Travel Position

Pay: $2,225/week
Job Description
We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Lewistown, MO, on a travel contract. The Physical Therapist will evaluate residents, develop tailored treatment plans, and deliver therapy services to enhance mobility, restore function, and promote independence. The ideal candidate is patient-centered, adaptable, and experienced in geriatric rehabilitation.
Key Responsibilities
- Conduct thorough evaluations to assess residents’ functional abilities and therapy needs.
- Develop and implement personalized treatment plans in coordination with physicians and care teams.
- Provide therapeutic interventions including strengthening, gait training, balance activities, and mobility support.
- Educate residents, families, and caregivers on therapy objectives, safety practices, and use of assistive equipment.
- Document evaluations, treatments, and progress notes accurately and promptly.
- Collaborate with nursing and therapy staff to ensure coordinated, patient-centered care.
- Support independence, safety, and improved quality of life through evidence-based therapy practices.
Qualifications
- Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Missouri (or compact license).
- Minimum of 1 year of recent PT experience, preferably in skilled nursing or long-term care.
- Current certification in Basic Life Support.
- COVID-19 vaccine and recent tuberculosis test.
- Strong clinical judgment, communication, and organizational skills.
- Compassionate approach with experience supporting geriatric populations.
