Pay: $2,095/week

 

Job Description:

We are seeking a skilled and compassionate Physical Therapist (PT) to join a skilled nursing and long-term care facility in Danbury, CT, on a travel contract. The PT will assess residents, design individualized treatment plans, and deliver therapy services that promote mobility, independence, and improved quality of life. The ideal candidate is patient-focused, organized, and experienced in geriatric rehabilitation.

 

Key Responsibilities:

  • Conduct detailed evaluations to assess patient mobility, function, and rehabilitation needs.
  • Develop and implement individualized treatment plans in collaboration with physicians and care teams.
  • Provide therapeutic interventions including strengthening, balance training, gait training, and functional mobility activities.
  • Educate residents, families, and caregivers on therapy goals, safe mobility techniques, and use of assistive equipment.
  • Document evaluations, progress notes, and outcomes in accordance with facility and regulatory requirements.
  • Collaborate with interdisciplinary healthcare teams to ensure comprehensive, patient-centered care.
  • Support residents in achieving greater independence, safety, and well-being.

 

Qualifications:

  • Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
  • Current and valid Physical Therapist license in Connecticut (or compact license).
  • Minimum of 1 year of PT experience, preferably in skilled nursing or long-term care facilities.
  • Current Basic Life Support (BLS) certification.
  • COVID vaccine and recent TB test.
  • Excellent communication, clinical assessment, and organizational skills.
  • Compassionate approach with experience supporting geriatric populations.
