Pay: $2,634.00/week
Completion Bonus: $2,500 per contract (up to $7,500 after three contracts)

Job Description

ATC Healthcare is seeking an experienced Home Health Physical Therapist (PT) for a travel assignment in Novato, California. This role provides in-home physical therapy services to adult and geriatric patients, focusing on functional mobility, rehabilitation, safety, and independence in the home setting.

The PT will deliver comprehensive home-based care, develop individualized treatment plans, perform functional assessments, and collaborate with interdisciplinary teams to support recovery and long-term patient outcomes. This position requires strong autonomy, critical thinking, and the ability to manage complex caseloads independently in a community-based care environment.

Key Responsibilities

• Perform in-home physical therapy evaluations and reassessments.
• Develop and implement individualized treatment plans.
• Deliver skilled therapy focused on mobility, strength, balance, and function.
• Conduct safety and environmental assessments in patient homes.
• Educate patients and families on exercises, safety, and care plans.
• Complete OASIS documentation and care coordination.
• Manage discharge planning and long-term goals.
• Collaborate with interdisciplinary care teams (PT, social services, home health staff).
• Maintain accurate and timely clinical documentation.

Qualifications

• Graduate of an accredited physical therapy program.
• Active California Physical Therapist license (required at submission).
• BLS certification (AHA or American Red Cross).
• Valid Driver’s License (Class C) and active automobile insurance required.
• Minimum 2 years of experience required (5 years recent relevant experience strongly preferred).
• Travel experience required.
• Home Health experience preferred.
• Strong independent clinical judgment and problem-solving skills.
• Excellent communication, organization, and time management abilities.
• Proficient with required technology and documentation systems.
