Shift: 5×8 Days (Monday–Friday, 8:00 AM–4:30 PM)
Pay: $2,560/week
Completion Bonus: $2,500 per contract (up to $7,500 after three contracts)
Position Overview
ATC Healthcare is seeking a skilled and compassionate Registered Nurse Case Manager for an acute-care facility in Paramount, California. The RN Case Manager coordinates the management of patient care to ensure quality outcomes, appropriate resource utilization, and seamless transitions across the continuum of care. This role integrates case management, utilization review, and discharge planning to enhance patient satisfaction, ensure regulatory compliance, and support efficient service delivery.
Key Responsibilities
- Conduct comprehensive patient assessments to identify medical, psychosocial, and discharge needs.
- Develop and implement individualized care plans, collaborating with the interdisciplinary team to ensure timely, safe, and effective transitions of care.
- Perform utilization review to ensure appropriate resource use, length of stay, and payer compliance.
- Coordinate discharge planning, including home health, SNF, rehab, and community service arrangements.
- Communicate regularly with physicians, payers, and external agencies to advocate for patient needs and resolve barriers to care.
- Provide patient and family education regarding treatment plans, post-acute options, and community resources.
- Maintain accurate and timely documentation in the EMR system.
- Serve as a patient advocate, ensuring that care aligns with patient goals and preferences while maintaining cost-effectiveness.
Qualifications
- Active California RN license (must be attached at submission; California is not a compact state).
- Minimum 2 years of recent RN Case Management and discharge planning experience in an acute hospital setting (required).
- Certifications: BLS (AHA only; must be attached to submission packet).
- Strong knowledge of utilization management, discharge planning, and payer guidelines (Medicare, Medi-Cal, and commercial).
- Excellent communication, critical thinking, and coordination skills.
- EMR proficiency (experience with Epic or Cerner preferred).
- Must be able to start within 2–3 weeks of offer.
- Proof of COVID-19 vaccination and recent tuberculosis test required.
