Pay: $2138/week
Completion Bonus: Up to $7,500 total. (You will receive $2,500 after the successful completion of each of your first 3 contracts with us)
Job Description:
We are seeking a highly skilled and resourceful Social Worker to join the Social Services team in an Adult Acute Care Hospital setting (approximately 344 beds). This critical role involves expert Case Management and discharge planning for patients with complex medical needs. The Social Worker will assess psychosocial needs, provide crisis intervention, coordinate discharge to various post-acute settings, and act as a vital bridge between the patient, family, and medical team. The role requires handling a high volume of complex cases and dedicated discharge planning. Proficiency in hospital discharge protocols and familiarity with utilization management systems are essential for success in this fast-paced environment
Key Responsibilities:
- Manage and execute complex discharge plans for a high volume of adult acute care patients (caseload up to 1:25-30 for discharge planning focus).
- Handle complex cases (ratio 1:8 or 1:9 when paired with a nurse matrix) requiring intensive psychosocial intervention and resource management.
- IParticipate in Multidisciplinary Rounds (MDR) or Interdisciplinary Team (IDT) meetings to communicate patient needs and facilitate appropriate disposition.
- Utilize electronic health record systems (including MIDAS/Meditech, Nate, and Navihealth) for accurate documentation, utilization review, and compliance with regulations including PASRR (Preadmission Screening and Resident Review).
- Provide crisis intervention, emotional support, and resource linkage to patients and families, addressing barriers to a safe transition.
- Must be willing to work every third weekend and be required to float to all units except Trauma to meet staffing needs.
Qualifications:
- Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW) is required.
- Minimum of 1-2 years of recent Social Work experience in an Adult Acute Care Hospital setting.
- Mandatory Experience: Proficiency with MIDAS/Meditech (EHR), PASRR (Preadmission Screening), and Navihealth (Utilization Review/Discharge Planning).
- Strong organizational, critical thinking, and conflict resolution skills.
- Must be adaptable and able to function well independently.
- COVID vaccine and recent TB test may be required.
