Job Summary

ATC Healthcare Services is seeking a compassionate and dedicated Social Worker to join our team in Farmville, VA. The Social Worker will support individuals and families by providing counseling, care coordination, and access to community resources. This role focuses on improving clients’ overall well-being, addressing social and emotional challenges, and helping individuals achieve stability and independence.

Key Responsibilities

• Conduct client assessments to identify social, emotional, and environmental needs

• Develop and implement individualized care and support plans

• Provide counseling and crisis intervention to individuals and families

• Connect clients with community resources, housing, healthcare services, and support programs

• Collaborate with healthcare providers, case managers, and community organizations

• Maintain accurate case documentation and reports in accordance with organizational policies

• Advocate for clients to ensure they receive appropriate services and support

• Monitor client progress and adjust care plans as necessary
  
  

Qualifications

• Bachelor’s or Master’s degree in Social Work (BSW/MSW) or a related field

• Active state licensure (if required in Virginia)

• Previous experience in case management, counseling, or community support services preferred

• Strong communication, empathy, and problem-solving skills

• Ability to work effectively with individuals from diverse backgrounds
