Key Responsibilities:
- Conduct assessments to identify clients' social, emotional, and behavioral needs.
- Develop and monitor individualized service/care plans.
- Provide supportive counseling and crisis intervention as appropriate.
- Coordinate referrals to healthcare, mental health, housing, financial assistance, and community resources.
- Advocate for clients and help them navigate available services.
- Maintain accurate and confidential client records and documentation.
- Collaborate with healthcare providers, case managers, schools, families, and other professionals.
- Participate in case conferences and multidisciplinary team meetings.
- Monitor client progress and make recommendations for additional services.
- Follow organizational policies and applicable federal, state, and local regulations.
Typical Qualifications:
- Master's degree in Social Work (MSW) from an accredited program.
- LMSW or LCSW preferred/required depending on the position.
- Strong communication, assessment, documentation, and interpersonal skills.
- Experience working with individuals, families, or vulnerable populations preferred.
- Knowledge of community resources and social service systems.
- Ability to maintain confidentiality and handle sensitive situations professionally.
