Key Responsibilities:

  • Conduct assessments to identify clients' social, emotional, and behavioral needs.
  • Develop and monitor individualized service/care plans.
  • Provide supportive counseling and crisis intervention as appropriate.
  • Coordinate referrals to healthcare, mental health, housing, financial assistance, and community resources.
  • Advocate for clients and help them navigate available services.
  • Maintain accurate and confidential client records and documentation.
  • Collaborate with healthcare providers, case managers, schools, families, and other professionals.
  • Participate in case conferences and multidisciplinary team meetings.
  • Monitor client progress and make recommendations for additional services.
  • Follow organizational policies and applicable federal, state, and local regulations.

Typical Qualifications:

  • Master's degree in Social Work (MSW) from an accredited program.
  • LMSW or LCSW preferred/required depending on the position.
  • Strong communication, assessment, documentation, and interpersonal skills.
  • Experience working with individuals, families, or vulnerable populations preferred.
  • Knowledge of community resources and social service systems.
  • Ability to maintain confidentiality and handle sensitive situations professionally.
