Job Description

ATC Healthcare of Arizona is seeking a qualified Social Worker for a 13 week contract assignment at a large hospital in West Prescott, AZ. This role is ideal for a compassionate, adaptable professional who thrives in fast‑paced clinical or community settings and is committed to supporting patients and families through high‑quality social services.

Key Responsibilities

• Assess patients’ psycho-social needs and develop appropriate care or discharge plans
• Provide counseling, crisis intervention, and resource coordination
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care
• Advocate for patients and assist with referrals to community resources
• Maintain accurate documentation in accordance with facility, state, and federal guidelines
• Support patients and families in navigating healthcare, social, and financial challenges

Qualifications

• Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited program
• Active Arizona Social Work license (or ability to obtain)
• Minimum of 1 year of social work experience (healthcare or facility‑based experience preferred)
• Strong communication, organizational, and advocacy skills
• Ability to adapt quickly to new environments and policies
