Summary

We are seeking dedicated Physical and occupational Therapists to join our team at ATC Healthcare. In this role, you will provide exceptional therapy services to patients in various settings, including outpatient and home health environments. Your expertise will be crucial in developing care plans and facilitating discharge planning to enhance patient recovery.

Responsibilities

• Evaluate and assess patients' physical abilities and limitations.
• Develop individualized treatment plans based on patient needs and goals.
• Implement therapeutic interventions, including trigger point therapy and rehabilitation exercises.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and families on treatment options and home health strategies.
• Document all patient interactions and maintain accurate records.

Requirements

• Valid state license to practice as a Physical or occupational Therapist.
• Experience in outpatient and home health settings preferred.
• Strong knowledge of discharge planning and care plan development.
• Excellent communication skills for effective patient interaction.
• Ability to work collaboratively within a multidisciplinary team.
