What You’ll Do:

• Transform Lives: Create and implement comprehensive care plans, enhancing the lives of elderly clients and helping them maintain independence.
• Lead with Expertise: Serve as the go-to expert for case management, helping families navigate the complexities of aging care.
• Make a Real Impact: Assess new clients, ensuring they receive the right services to enhance their quality of life and well-being.

What We’re Looking For:

• Registered Nurse (RN) with an active license in the state of WV.
• Prior experience in geriatric care management and/or home care is required.
• A compassionate heart and a desire to work with seniors.
• Excellent communication and organizational skills to handle multiple cases.
• Ability to travel frequently between Huntington and South Charleston.
• A team player who thrives in a supportive, collaborative environment.
