Responsibilities

  • Serves as a liaison between family, the resident, the facility and the referring facility in order to make a smooth transition into the facility.
  • Provides education to the resident and the family as to expectations and what psychological and social changes will take place.
  • Assists in the marketing of the facility. Performs presentations and attends functions on behalf of the facility to keep a positive reputation in the community.
  • Acts as a case manager on behalf of the resident. Performs counseling as needed.
  • Assists in the documentation efforts of the facility. Provides adequate documentation for all services performed.
  • Assists in developing and implementing policies and procedures for the identification of medically related social and emotional needs of the resident.
  • Participates in community planning related to the interests of the facility and the services and needs of the resident and family.
  • Participates in discharge planning, development and implementation of social care plans and resident assessments.
  • Provides consultation to members of our staff, community agencies, etc., in an effort to solve the needs and problems of the resident through the development of social service programs.
  • Provides solution for social and practical problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
  • Interprets social, psychological, and emotional needs, of the resident/family to the medical staff, attending physician, and other resident care team members.
  • Develops, implements, and maintains an ongoing quality assurance program for the social services department.
  • Ensures that all charted progress notes are informative and descriptive of the services provided and of the resident's response to the service.
  • Develops preliminary and comprehensive assessments of the social service needs of each resident.
  • Develops a written plan of care (preliminary and comprehensive) for each resident that identifies the problem/needs of the resident and the goals to be accomplished for each problem/need identified.
  • Performs other duties as assigned. 

Qualifications

  • Graduation from a accredited four-year college or university with a degree in social work, sociology, or closely related degree.
  • Two years of experience in long term care. 
  • Knowledge of social work theory, principles, methods, techniques; and of the dynamics of human behavior.
  • Ability to identify dysfunctional relationships and environment conditions.
  • Knowledge of Medicare, Medicaid, veterans, managed care, public administration, and insurance payer requirements.
  • Ability to assess social service needs and identify programs or services sufficient to meet client needs.
  • Ability to read, write, speak, and understand the English language.
  • Ability to make independent decisions when circumstances warrant such action. 
  • Ability to relate information concerning a resident's condition.
  • Ability not to pose a direct thereat to the health or safety of other individuals in the work place.
  • Skill in the use of the sight/hearing senses or use of prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  • Ability to meet the general health requirements set forth by the policies of this facility, including a medical and physical examination.
  • Ability to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of three feet and be able to push, pull, move, and/or carry such weight a minimum distance of three feet.
