Community Partnership Liaison

America’s Home Health Services is expanding in Philadelphia and seeking a driven, relationship-focused professional to help grow our community presence and referral network.

This is a structured growth role with clear accountability and measurable outcomes — directly tied to market expansion and revenue impact.

What You’ll Do:

• Develop and strengthen referral source partnerships

• Build relationships with community organizations

• Educate families on Community HealthChoices (CHC) and waiver services

• Generate private pay introductions

• Represent America’s Home Health Services at community events

• Track and report measurable growth metrics

• Highly motivated self-starter

• Strong ties to Philadelphia communities

• Comfortable presenting and building strategic partnerships

• Goal-oriented with a performance mindset

• Healthcare, outreach, or sales experience preferred
