Description of the Role:
We are currently seeking a highly skilled and passionate Physical Therapist to join our esteemed team at Always Best Care Senior Services - Oahu. This is an incredible opportunity to have a positive impact on the lives of our valued patients in the stunning city of Honolulu, Hawaii.
Responsibilities:
- Provide exceptional, personalized care to patients in the comfort of their own homes
- Conduct comprehensive assessments to determine patients' individual needs
- Develop and modify personalized treatment plans based on patient progress
- Contribute to the development and implementation of interdisciplinary patient care plans
- Collaborate closely with physicians to administer diagnostic and prognostic tests
- Utilize a diverse range of physical therapy techniques to alleviate pain and enhance function
- Educate and guide patients and their families in the physical therapy program
- Coordinate outpatient services for procedures that cannot be administered at home
- Maintain accurate and timely records of evaluations, treatments, and patients' therapy intervention responses
- Provide supervision to Physical Therapy Assistants and paraprofessionals
Requirements:
- Graduate of a Physical Therapy school approved by the Council of Medical Education of the American Medical Association, in collaboration with the American Physical Therapy Association
- Valid license to practice Physical Therapy in the state of Hawaii
- Minimum of one year of institutional/home health experience (preferred)
- Complete criminal background check and current CPR certification
