We are seeking an experienced and compassionate Health Services Director / Director of Nursing to lead the Wellness department within our assisted living and memory care community.
This position is responsible for the overall coordination and oversight of resident health services, nursing operations, medication management, resident assessments, service planning, clinical documentation, staff development, and regulatory compliance.
This is a highly visible leadership position that works closely with residents, families, providers, pharmacies, hospice agencies, community leadership, and the resident-care team to ensure residents receive appropriate, individualized, and compassionate support.

What You'll Do
Resident Health & Nursing Services
-
Provide leadership and oversight of the community's Wellness and resident-care services
-
Monitor the overall health and wellness needs of residents
-
Complete and oversee resident nursing assessments and required reviews
-
Identify changes in resident physical, cognitive, behavioral, and functional status and ensure appropriate follow-up
-
Participate in the development, implementation, review, and updating of resident service plans
-
Ensure resident services remain appropriate based on each resident's current needs and the community's level of care
-
Coordinate resident care with physicians, advanced practice providers, pharmacies, hospitals, rehabilitation providers, home health agencies, and hospice providers
-
Review hospital and rehabilitation returns and help coordinate safe transitions back to the community
-
Provide nursing consultation and clinical guidance to community leadership and resident-care staff
-
Respond to significant changes in condition and ensure appropriate escalation, documentation, and communication
-
Support residents and families through changes in health status and level-of-care needs
-
Promote resident independence, dignity, choice, safety, and quality of life
Assessments & Service Planning
-
Complete or oversee nursing assessments as required
-
Participate in determining whether resident needs can be safely and appropriately supported within the community

Review resident health information prior to and following move-in
-
Partner with Sales and community leadership during the assessment, admission and move-in process
-
Ensure appropriate follow-up when a resident experiences a significant change in condition
-
Participate in service-plan meetings and communicate changes to appropriate team members
-
Maintain accurate and timely nursing documentation
-
Identify when a resident may require additional services, outside providers, or a different level of care
Medication Management
-
Oversee medication-management practices within the community
-
Ensure medications are administered or assisted with according to provider orders, applicable law, community policy, and the community's licensing level
-
Review medication orders and medication records for accuracy and appropriate follow-up
-
Coordinate medication concerns with pharmacies and prescribing providers
-
Monitor medication errors, omissions, refusals, adverse reactions, and other medication-related concerns
-
Conduct medication-cart, medication-record, and documentation audits
-
Ensure medication storage, security, documentation, and disposal procedures are followed
-
Provide oversight and required evaluation of medication aides and other authorized medication personnel
-
Identify medication-management trends and implement corrective action or additional training when needed
Wellness Team Leadership
-
Lead, support, and develop the Wellness and resident-care team
-
Participate in recruiting, interviewing, hiring, on-boarding, and retention of Wellness employees
-
Assist with staffing plans and ensure appropriate resident-care coverage
-
Provide direction, coaching, and performance feedback to nursing and resident-care employees
-
Monitor employee attendance, performance, competency, and compliance with community expectations
-
Ensure employees receive required orientation, training, competency reviews, and continuing education
-
Promote accountability, teamwork, communication, and a resident-centered culture
-
Ensure staff understand resident service plans, care needs, safety concerns, and required interventions
-
Provide appropriate nursing supervision and delegation in accordance with applicable law and nursing standards
-
Maintain appropriate documentation of employee training and competencies
Quality, Compliance & Risk Management
-
Help ensure Wellness operations comply with applicable assisted living regulations, nursing requirements, and community policies
-
Maintain required nursing, resident-care, and clinical documentation
-
Participate in quality improvement activities and identify opportunities to improve resident outcomes and service delivery
-
Review incidents, falls, medication errors, hospital transfers, infections, wounds, and other clinical trends
-
Develop and implement corrective actions when needed
-
Participate in regulatory surveys, inspections, audits, and plans of correction
-
Ensure resident rights, privacy, dignity, and confidentiality are protected
-
Monitor infection prevention and control practices
-
Support emergency preparedness and resident safety initiatives
-
Ensure clinical concerns are appropriately communicated to the Executive Director and other members of the leadership team
Resident & Family Communication
-
Build strong relationships with residents and families
-
Serve as a clinical resource for resident and family questions
-
Communicate professionally and compassionately regarding changes in condition, level of care needs, and health-related concerns
-
Participate in resident and family meetings as appropriate
-
Address concerns promptly and work collaboratively toward resolution
-
Help families understand changes in resident needs and available care options
Hospice & Outside Provider Coordination
-
Coordinate with hospice, home health, therapy, pharmacy, and other outside healthcare providers
-
Ensure outside services are appropriately documented and communicated to the community team
-
Support residents and families receiving hospice or end-of-life services
-
Promote coordination between community staff and outside healthcare professionals
-
Ensure services provided within the community remain consistent with applicable law and the community's licensing level
What We're Looking For
-
Current and active nursing license in good standing as required by the state and position
-
Previous nursing experience working with older adults
-
Assisted living, memory care, long-term care, home care, or similar experience strongly preferred
-
Previous nursing leadership, supervisory, or management experience preferred
-
Strong knowledge of geriatric nursing and the aging process
-
Knowledge of medication management, dementia care, fall prevention, infection prevention, hospice services, and common chronic conditions affecting older adults
-
Strong resident-assessment and clinical-monitoring skills
-
Ability to recognize and respond appropriately to changes in resident condition
-
Experience supervising, coaching, and developing nursing or resident-care staff
-
Strong knowledge of nursing documentation and service planning
- Proficient in Microsoft Office, including Word, Excel, and Outlook
- Experience with applicant tracking software or the ability to learn it quickly
- Able to manage supply ordering and inventory tracking within budget
- Experience processing bi-weekly/semi-monthly payroll accurately and on schedule

Responsibilities requiring a specific licensed professional must be referred or escalated appropriately

Delegation must consider the resident's condition, the nature of the task, employee competency, and the level of supervision required

Required nursing documentation, medication records, assessments, and clinical records must be maintained accurately and timely
